| STEP 1: SUBMIT PROPERLY FILLED-OUT APPLICATION FORM WITH REQUIREMENTS TO BPLO CUSTOMER SERVICE COUNTER FOR SCREEENING OF DOCUMENTS, AND WAIT FOR YOUR BILLING. |
For INCOMPLETE requirements, please attach Oath of Undertaking.
Required Documents:
- Application Form
- Barangay Clearance
- Zoning Clearance for NEW business application
- Lease of Contract (if applicable)
- Occupancy Permit for NEW business application
- Business Registration, whichever is applicable:
- DTI Certification for Single Proprietorship
- SEC Registration for Partnership, Corporation, Cooperative & Association
- CDA Accreditation for Cooperative only
- Sketch Location of business showing nearest/important landmarks
- Comprehensive General Liability (CGL) Insurance
- Latest Photos of the following:
- 1 – Front View showing signboard or business name
- 1 – Right or Left View showing property line & signboard, if applicable.
- 1 – Inside photo of establishment
- 1 – Photo of declared owner or corporate logo, whichever is applicable.

| STEP 2: PROCEED TO THE OFFICE OF THE CITY TREASURER FOR PAYMENTS OF CEDULA, REGULATORY FEES AND BUSINESS TAXES. |

| STEP 3. SUBMIT OFFICIAL RECEIPT (O.R.) & DOCUMENTS TO BPLO RECEIVING SECTION FOR INSSUANCE OF CLAIM STUB AND VALIDATION OF O.R. AS TEMPORARY PERMIT. |
Note:
- Application with incomplete documents will not be accepted for processing.
- Ocular inspection will be conducted prior to releasing of business permit for NEW application only.
- For RENEWAL of application, inspection follows after Step 4.

| STEP 4. RELEASING OF BUSINESS PERMIT AND OTHER LICENSES. |
Note: Business Permit and other licenses will be release after seven (7) days upon O.R. validation (Step 3) from Receiving / Releasing Section of BPLO. |
