.: Requirement for Certificate of Live Birth |

| BIRTH CERTIFICATE |
| 1. All events of birth are registered in the Civil Registry Office within thirty (30) days. |
| |
| 2. If married parents, Marriage Contract is needed (CERTIFIED TRUE COPY) |
| |
| 3. If not married, Affidavit to Use the Surname of the Father (AUSF) will be attached. |

| 4. The Hospital / Clinic Administrator, the attendants at birth (hilot, midwives, OB-Gyne) or the parents themselves shall report such event. |
| |
| 5. If beyond thirty (30) days it is considered delayed. |

| DELAYED REGISTRATION OF BIRTH |
1. Four (4) copies of the Certificate of Live Birth duly accomplished and signed by the proper parties;
- Strictly No Erasures
- The OCRG Copy must be printed legibly

| 2. Duly accomplished Affidavit for Delayed Registration (at the back of the Certificate of Live Birth) signed by the father, mother or guardian, or the child himself, if he is of understanding capacity; |
3. Original NSO & CRO Certification "No Record on File". |
4. Any two (2) of the following documents to prove name, date, place of birth, & parents of the registrant.
- Original Baptismal Certificate
- Certified Copy of form 137 / School Records ( High School, Elementatry, Nursery, Kinder or Prep)
- Certified Copy of the registrant's Medical record
- Income Tax Return of parents or registrant
- Certified Copy of Voter's Affidavit, if a voter
- Certificate of No Marriage (CENOMAR) of both parents, if child was born before the marriage.
- Employment Service Record, if employed
- Insurance policy, if insured
- Affidavit of Two (2) Disinterested Persons who might have witnessed or have known about the birth of the child.
- The least acceptable supporting documents, if only one of the above is available, Barangay Captain's Certification.

| 5. Duly accomplished Affidavit of Acknowledgement / Admission of Paternity, if the child is illegitimate. (If Father is abroad must execute an Affidavit thru the nearest Philippine Consulate at its residence) |
| 6. For those born and registered outside Metro Manila, execute an Affidavit of Out-Of-Town Reporting attested by two (2) persons. |
