MARRIAGE LICENSE APPLICATION

1. Certified Copy of Birth / Baptismal Certificate - Bring Original

2. Residence Certificates

3. Parental Advice (21 yrs. - below 25 yrs. old)

4. Parental Consent (18 yrs. - below 21 yrs. old)

5. Pre - Marriage Counseling from DSWD and MHD

6. Divorced Paper / Annulment Paper (if Divorced / Annulled)

7. Death Certificate (if Widow / Widower)

8. Barangay Certificate (for Taguig residents /s)

9. Legal Capacity to Marry issued by the Embassy

10. Xerox copy of Foreigner's Passport

11. CENOMAR from NSO for Filipinos Only

Note: Application for Marriage License must be accomplished in own handwriting applicants. LEFT column for MALE and RIGHT for FEMALE.

APPLICATION FOR DELAYED REFGISTRATION OF MARRIAGE

1. NSO & CCRO Certification of "No Record on File"

2. Original copy of unregistered Marriage Contract with signatures

3. Affidavit for delayed registration executed by the solemnizing officer or the person reporting or presenting the certificate of Marriage, the facts and circumstances surrounding the marriage and reason or cause of the delay.

4. In the absence of old Marriage Contract, certified copy of solemnizing officer's record / registry book.

5. Affidavit of secretaries, witnesses and contracting parties.

6. Certified copy of the reconstructed marriage license or certification that license was issued.

7. Four (4) copies of Certificate of Marriage which must be accomplished correctly and completely.
