  5. Original or copy of previous Business Permit

  9. Certificate of Gross Receipts for the previous years (signed by the President and Treasurer)
  10. Audit Financial Statements from the previous years (stamped “RECEIVED” by the BIR/or
  11. Quarterly VAT Returns – 1st Qtr. to 3rd Qtr. (BIR Form 2550Q) or Quarterly Percentage Tax Returns (BIR Form 2551Q)/or
  12. Monthly VAT Returns (BIR Form 2550M) or Monthly Percentage Tax Returns (BIR Form 2551M)

  1. Original Business Permit (5 yrs backward).
  2. Photocopy of Business Tax Receipt (5 yrs backward).
  3. Affidavit of Business retirement Duly Notarized.
  4. Original / Barangay Certificate of Closure.
  5. Books of Account (5 yrs backward).
  6. Photocopy of VAT Returns / ITR (5 yrs backward).
  7. Original receipt issued by BIR / Sales and Delivery Invoices.
  8. Audited Financial Statement (5 yrs backward).
  9. Sketch of business Location.
  10. If you are not the owner. Authorization letter from the Owner with photocopy of Valid ID from the owner.

REQUIRMENTS FOR CORPORATION

  1. Original Business Permit (5 yrs backward).
  2. Photocopy of Business Tax Receipt (5 yrs backward).
  3. If you have other branches or consolidated, submit business tax permit and mayors permit from that municipality. Photocopy only.
  4. Original / Barangay Certificate of Closure.
  5. Original secretary’s Certificate / Board Resolution Business.
  6. Photocopy of VAT Return (5 yrs backward).
  7. Books of Account (5 yrs backward).
  8. Audited Financial Statement (5 yrs backward).
  9. Schedule of Gross Sales / Receipts for everyday Branch Duly Certified by an Authorized Official of Establishment.
  10. Original receipt issued by BIR (Used and unused).
  11. Photocopy of Authority to print coming from BIR
  12. Authorization latter to those who will process the Docs.
